Access and Retrieval of Your Records

Once we receive your request for your medical records, K.I.S. can send your file via any source the receiving facility has. All the receiving facility needs to have is either a fax, computer modem, or just a phone. We’ll make it happen. You will grant access by presenting your K.I.S. CARD and your password.

Access to your medical records may be achieved via 5 different methods.

You request your files by phone, fax, or computer. Examples are:
An emergency request
Medical records sent to your physician
Copies sent to your home.


You can provide access to your physician by providing him with your personal code information.


If you’re not concerned about complete privacy (i.e. a person in a convalescent home or hospital) your password can be printed on your ID.


Restrict access to specifically named people.
Examples would be your parents or children, or a person with Power of Attorney, or your personal physician.


Change your access code anytime you desire, and you can restrict privileges at any time.

    Retrieval of Medical Records
As soon as your data is collected and your medical history file is complete, your records are ready at a moment’s notice. Once authorization is complete your files can be sent anywhere in the world. The entire process takes only a few minutes. The request for your medical file can be as often as necessary at no additional charge.
